The team is able to organise itself and work together, but we have situations where unexpected dependencies are discovered which were not appreciated during backlog refinement. | |

A person at the end of the blocker being accused of the blocker doesn't sound like psychological safety to me, but lack of it. I suggest you find ways for the team to work together to unblock a situation that to call that situation by another name. | |
